That most cancers strike men more often than women isn?t news to the medical world.? Known risk factors, such as smoking, drinking or occupational hazards may explain some of the gender difference in contracting the disease.? But more than one-third of the cancers that disproportionately strike one sex or the other?men, in particular?can?t be explained by known risk factors.

Research leader Gustaf Edgren, a research fellow in the Harvard School of Public Health?s Department of Epidemiology, says the differences seem to be associated with gender alone.

According to the World Health Organization, there were 12.7 million new cancer cases around the world in 2008.? 6.6 million occurred in men and 6 million in women.? The total number of cases is expected to increase to 21 million by 2030.

Edgren, senior author Ellen Chang of Stanford University Medical School and researchers from the Karolinska Institute in Stockholm postulate that puzzling sex disparities in cancer rates are likely the result of intrinsic, sex-specific biological factors or a difference in susceptibility to risk factors.

They believe that if the causes of the disparities were identified and were modifiable, more than one-third of cancers could be prevented.

For their study, the researchers analyzed almost 15 million cases of cancer in 60 countries from a worldwide database of cancer statistics, Cancer Incidence in Five Continents.

?Not only do men have an increased risk of 32 of the 35 most common cancers, but intriguingly, for many of those cancers the male excess risk is very strong.? The risk has remained almost constant since at least the 1960s, and is completely enigmatic,? said Edgren.? ?We simply have no idea why.?

The research team found that for nearly half of the 35 cancers examined, men contracted the disease twice as often as women.? The male risk was fourfold in five of the cancers:? cancers of the larynx, hypopharynx, and lip; urinary bladder cancer; and Kaposi sarcoma.

For 13 of the 35 cancers, 12 of which strike men more often than women, they found no known risk factors that could plausibly explain the sex disparities.? Those disparities remained consistent over four decades.? Some of the cancers included thyroid cancer, myeloid leukemia, multiple myeloma and non-Hodgkin lymphoma.

?While the sex disparity for most of these cancers is fairly well-known, no one has really taken a holistic view of this topic before,? Edgren said.? ?Both within science and outside, I?ve yet to come across anyone who is aware of just how large these differences are.? Here we show that cancer is first and foremost a disease of men.?

But what about the other HTC One S?

As it turns out, there's a second version of the No. 2 phone in HTC's 2012 lineup. The internals are the same, but it's traded in the deep black MAO finish for a more traditional metallic paint job. HTC calls it a "gradient paint color sheet," and it's got a decidedly more traditional feel. It's not bad, just different, with more of a smooth finish. The silver color is straight out of the old-school HTC playbook as well, and we'd be more than welcome to find a place in our pockets for either one of these phones.

While controversy over one aspect of?the Obama administration?s contraception rule ? whether and when religiously affiliated employers must comply ? has dominated recent headlines, that debate has obscured other questions about how the?rules will actually be implemented.

Under the health law, insured women will qualify for contraceptives without a copaymentas part of a range of preventive medical services. But insurers and advocates are frustrated by the lack of details. They?re asking, for example, whether surgical procedures must also be covered at no additional cost to patients.

?The unknown answers to the myriad of questions remain very worrisome,? says Judith Lichtman, senior advisor to the National Partnership for Women and Families, an advocacy group in Washington D.C.

America?s Health Insurance Plans (AHIP), the industry lobbying group, sent a list of questions to the Department of Health and Human Services (HSS) in September, asking for clarification on what types of contraceptives must be covered and how insurers should structure their policies. A spokesman for AHIP says the group is still awaiting answers to some questions.

Contraceptives are part of a package of women?s preventive health care services under the Affordable Care Act ? including screening for diabetes, counseling about sexually transmitted diseases and breast feeding support ? that must be provided without deductibles or co-pays in all new policies beginning Aug. 1. The package was drawn up after recommendations from the Institutes of Medicine (IOM). Other parts of the health law also provide no-cost-sharing preventive services for children and all adults, such as certain?cancer screenings and immunizations.

Here are some questions, and answers when we could determine them, about the rules:

1) Are male-based contraceptive methods, such as vasectomies or condoms, covered by the rule?

An HHS official said on Friday that women?s preventive services guidelines apply to women only.

Guidelines issued by the Health Resources and Services Administration, part of HHS, require coverage without cost sharing for ?all Food and Drug Administration-approved contraceptive methods, sterilization procedures and patient education and counseling for all women with reproductive capacity? as prescribed by a provider, according to the Federal Register.

The insurers? letter from September says they interpreted the rule to include only female-based contraception and that the requirement to waive co-payments ?does not apply to methods and procedures intended for males.?

But Adam Sonfield, senior public policy associate at the Guttmacher Institute, a reproductive health research group, says the language is unclear, and it would be foolish to exclude vasectomies. For one thing, he says, they are less expensive and pose a lower risk of complications than female surgical sterilization methods. Plus, he says, waiving co-payments for services for one sex but not the other raises issues of discrimination.

?I can?t see how it would be in anyone?s interest to treat them differently,? says Sonfield.

2) Are over-the-counter products like female condoms, spermicides, sponges covered by the rules and, if so, will they require a prescription and how will insurers reimburse policyholders for purchases at retail stores?

Products that must be covered without cost-sharing include over-the- counter contraceptives when they are prescribed by doctors, the HHS official said Friday. But getting a prescription for such items raises other issues, say advocates and insurers.

Insurers wrote HHS in September that ?it is unclear what specific over-the-counter products are to be included.?

In addition, the letter warned that requiring a prescription for such items ?would increase the burden on an already over-burdened primary care system and drive up administrative costs.?

Insurers wrote the industry has no simple way to track and reimburse policyholders who purchase those items at retail stores. Setting up such systems, ?could cost tens of millions? and exceed the cost of the products themselves.

Requiring a prescription should definitely not be required, says Lichtman, who said such a move would make using contraception more difficult and expensive for women.

3) If a hospital stay is required for surgical procedures, such as when a women gets her tubes tied, would the procedure be covered without cost sharing?

Insurers say it is not clear if hospitalization or complications that might occur from surgery would be covered without the patient paying a co-payment or a deductible because they are not considered preventive.

Sonfield at Guttmacher argues they would be covered.

?It?s like saying you have to cover blood transfusions, but not the hospital stay,? he says. ?That doesn?t make sense. If you?re covering it, you?re covering it all.?

HHS has not weighed in on this question, but is expected to issue additional guidance in the coming months.

4) Will insurers be required to cover all products in a class, such as all IUDs, or all birth control pills? Can insurers require a co-pay for a brand-name drug if a generic is available?

Many insurers have ?tiered? pharmacy benefits under which patients pay differing amounts for brand-name, as opposed to generic, products. Some require patients who choose a brand-name drug, when an equivalent generic is available, to pay the price difference between the two. Insurers say HHS guidance allows them to use such ?reasonable medical management? to help control costs. That would include allowing insurers to charge patients for brand name drugs, it says.

The HHS official confirmed that, but stressed the plan must ?accommodate any individuals for whom it would be medically inappropriate by having a mechanism for waiving the otherwise applicable cost-sharing for the branded version.?

Advocacy groups and insurers are in discussions with HHS over those and other questions related to preventive care, says Lichtman. Her group hopes the agency will soon release additional guidelines that ?are broad enough so that all methods prescribed by doctors necessary for women?s health will be covered.?

5) Who will be covered for contraceptives without co-payments?

The package of women?s preventive care benefits must be offered in all new insurance policies sold to individuals and employers starting Aug. 1, as well as in most policies that renew afterwards on the date that they renew. There is an?exception?for insurance provided by certain nonprofit religious employers who object to birth control.

Twenty-eight states already require insurers to cover contraceptives, although large, self-insured employers are generally exempted from state rules. But a 2010 Kaiser Family Foundation/HRET?survey?of employers found that 85 percent of large firms offered prescription contraceptives in their plans, although they often required a patient co-pay or deductible. (KHN is a program of the foundation)

As for the types of procedures covered, the IOM cites a 2011 Bureau of Labor Statistics?analysis?of 3,900 employer plans, which found that policy documents representing about 70 percent of participants did not mention sterilization. But when sterilization services were mentioned, nearly 90 percent of the policies covered both female and male surgical sterilization procedures.

A Shrink's Guide To Self-Publishing (part 2) | Private Practice Toolbox

In part 2 of ?A Shrink?s Guide To Self-Publishing? Stephanie Adams, LPC walks you through the process of organizing and completing your book and preparing it for publishing. Read part 1 here.

Last week, we talked about how self-publishing can be a shrink?s dream second income AND do wonders for their practice by increasing awareness of their expertise. But at the same time many of you who might consider this path worry that no matter how much you like the idea in general, you could never actually write a whole book.

I?m telling you now, don?t sell yourself short! Think about it. What is therapy? A large part of counseling is simply listening to the client?s story and creating together a different, better story of a life they can choose to lead. Probably at least once a session, you will find yourself doing things like attributing meaning to an event in the client?s past, explaining a difficult concept to them, teaching a technique, or painting a verbal picture. That?s storytelling, and you can do it on paper just as easily as you can do it in person!

What?s Your Book About?

This is not an answer you come up with on a purely intellectual level. One might say all writers are intimately tied to our material. But since the counselor?s vocational pursuits are so deeply personal, our stories will be even more so in many ways. So instead of coming up with a topic out of thin air, turn inward. What is your message to others? A practical way to start this process of discovery is by writing down things you say to clients every day. This will start to tell you what you are passionate about, and what you desire your clients to learn from you. By the time you?re finished with that initial burst of energy, you should have 2-3 pages of handwritten ideas?or more! As you look over these pages, you will start to see a theme take shape from even seemingly disconnected ideas.

The easiest way I?ve come up with to find your theme is something I call ?chunking.? It involves taking your brainstorming sheets and breaking into ?chunks? the ideas that go together. For example, say you?ve written, ?dancers, artists, art therapy, painting, family therapy, eating disorder.? Which of these go together? The answer to that question IS your theme. If you find yourself creating from these pages a mental narrative that involves the local school for the arts, eating disorders, and new techniques you?ve discovered, then it sounds like you?ve got an idea you want to share about a new method art students can use to conquer an eating disorder.

My ?theme? is pure conjecture ? but that is why I shared it. You will come up with a completely different idea from the same pages of notes than another person will. It?s not important that you include all the elements you wrote down on your brainstorming sheets, or that you say what you think someone wants to hear about. What?s important is that you discover the idea you?re meant to share.

Once a theme has become clearer to you ? because it WILL evolve as you get into the work of writing a book ? you must ask yourself if you need to narrow or broaden your topic to do the subject justice. This is another highly subjective area, but I have confidence you can come up with the right choice for you. Take the example above. There are many art students out there, and many different kinds of therapy for eating disorders. So what should you focus on?

Let?s say that you have a background in painting, and that is what you are most qualified to comment on. At the same time, you realize that dancers are more susceptible than other kinds of art students to eating disorders because of the focus on body image. Now, you could write about all the different groups you conduct, and all the kinds of therapy you are qualified to perform. But, you could do the most good writing a self-help guide to painting female figures in order to help dancers recover a healthy body image. This is the most precise definition of who you can help, and that is what is important.

If you cannot identify your reader, they will not be able to identify themselves as someone who can benefit from reading your book.

Once you have identified your reader, ask yourself what sets your book idea apart to the point that this potential reader will pick it up and say, ?Hmm, I?ve never heard about this quite this way before.? In order accomplish this result; you have a narrow range of specific options. The first option is to have a new piece of information no one has discovered before, which is hard to do in this day and age. The second option is to focus on an aspect of a problem no one has covered before, and the third is to deliver the information in a way no one has done before. How will you make your book stand out?

The Book Takes Shape

Flip back to the first few pages of notes you made, and take a moment to elaborate on the ideas you?ve started there. When you?ve done some more free writing, look over what you?ve come up with and start ?chunking? again. This time, of course, you?re starting from a narrower premise, so your specific chunks will become not book ideas, but chapters of your existing book idea.

Look at what you have. Maybe one cluster of ideas explains why this issue you?re circling around is a problem. Another cluster of ideas tells your potential reader what will and won?t work when addressing this issue.

Is it sounding a little less scary? I hope so! Writing nonfiction, like therapy, is just telling a client about a problem you see and how you?d like to help them overcome it.

Speaking of nonfiction, as you?re allowing your chapters to take shape, start thinking about what kind of nonfiction book best suits the material you?re sharing. Should you write a self-help book, generally written in the second-person and including exercises for the reader to work through on their own? Might you be writing a textbook for classroom use, or a training manual? Maybe your book would be best served as an inspirational journal? Or what if you?re a play therapist, and your child clients would benefit from a children?s picture book?

This is your book! Create the one you want!

Preparing Your Book For Publication

We?ve already talked a great deal about the process of formatting and submitting a book to be self-published through a service, but before then you want to make sure has been edited to the best of your ability. I don?t recommend doing this completely on your own. If you can afford a line editor, great?but they are quite pricey. A less expensive option is to join a writer?s group and exchange manuscript editing services with others in the group that are seeking publication.

Of course, with this kind of group, you will get varying levels of editing ability. That?s why if you?re going with lay editors, it?s best to allow for each individual editor to help you in their area of strength. For my book, I had two or three people help me read the book for accuracy within my field. I had someone who knew my style of writing to check for clarity. Another helpful reader pointed out cohesiveness. And I was also fortunate enough to have someone read my book who was an expert in grammar and punctuation.

Almost even more than the content, your readers will judge your professionalism by the level at which your material is presented. If your book is grammatically accurate, focused, and easy to understand, you will rise in their esteem. If your book is instead riddled with mistakes and they can?t understand what it?s about, your venture into self-publishing could end up hurting you instead of helping you.

Promoting Your New Expertise

After your book has been released, share it with people! Start a blog book tour, such as the one this article was written for. Send an announcement to your email list and share it with members of the community. Display it in your office, and have business cards printed up with the information on how to purchase your book. Organize a workshop or small group around your topic, and hand-sell the book afterwards.

Remember, writing a book says you?re serious about your topic, so you want to encourage everything that puts you closer to becoming the go-to person within your niche. Don?t worry ? you will still get a chance to explore other aspects of your career in the future, but for now you want to recede into your niche and stay there! You are cultivating an expertise, remember?

When you love the topic you wrote about, sharing it can be a joy, because you?re talking about something you love. Be creative, and have fun with it.
